At Case Management Cymru we are able to provide a highly professional and individualised specialist case management service to adults and children with a range of complex serious injuries.

We pride ourselves on providing bespoke services, recognising the uniqueness of every client; each having very different aspirations and challenges.

We are looking for a warm, patient and reliable individual to provide support and facilitate the independence of an 9-year-old girl, who has Cerebral Palsy, epilepsy and is registered as partially sighted.

Having excellent communication skills and maintaining a consistent and proactive approach you will assist our client, who lives at home with her mum, to engage in activities such as the leisure centre, horse riding and to attend community events. (The ability to drive using the family vehicle is essential). You will also work with independent therapists in enabling our client to participate in effective neuro rehabilitation.

Whilst experience in this area of work is preferred, it is not essential as a full induction and training will be provided.

This role is subject to an Enhanced level DBS check.
